Use this checklist at every rostered shift change, unplanned deployment, or extended operational period to maintain continuity and prevent information loss during high-consequence incidents.

Note: Completed handover checklists must be retained for audit and incident investigation purposes. File in ERT logbook or electronic record system. In the event of an active emergency, conduct verbal handover immediately and complete documentation when operationally safe to do so.
